Our Legal Duty
Orderly Wellness Corp (“we,” “us,” or “our”), operating OrderlyMeds, is required by law to maintain the privacy of your protected health information (“PHI”), to provide you with this Notice of Privacy Practices, and to follow the terms of this Notice currently in effect.
We reserve the right to change our privacy practices and the terms of this Notice at any time. Changes will apply to all PHI we maintain. Updated Notices will be posted on our website and made available to you upon request.
How We May Use and Disclose Your Health Information
Treatment
We may use and disclose your PHI to provide, coordinate, or manage your healthcare and related services. This includes sharing information with physicians, pharmacists, or other healthcare providers involved in your care through our telehealth platform.
Payment
We may use and disclose your PHI to obtain payment for services provided to you, including billing and collections activities, insurance verification, and prior authorization requests.
Healthcare Operations
We may use and disclose your PHI for our healthcare operations, including quality assessment, employee training, accreditation, and other activities that support the operation of our telehealth pharmacy services.
Required by Law
We may use or disclose your PHI when required to do so by federal, state, or local law, including for public health activities, law enforcement purposes, judicial proceedings, and national security purposes.
Business Associates
We may share your PHI with third-party “business associates” (e.g., billing vendors, IT providers) that perform services on our behalf. We require business associates to protect the privacy of your PHI under a written agreement.
Your Rights Regarding Your Health Information
Right to Inspect and Copy
You have the right to inspect and obtain a copy of your PHI in our records. We may charge a reasonable fee for copying. Requests should be submitted in writing to our Privacy Officer.
Right to Amend
If you believe your PHI is incorrect or incomplete, you may request an amendment. We may deny the request under certain circumstances permitted by law.
Right to an Accounting of Disclosures
You have the right to request a list of certain disclosures we have made of your PHI, other than for treatment, payment, and healthcare operations.
Right to Request Restrictions
You may request that we restrict how we use or disclose your PHI. We are not required to agree to all restrictions, but we will consider your request.
Right to Confidential Communications
You may request that we communicate with you about your health information in a specific way or at a specific location (e.g., only by email at a specific address).
Right to a Paper Copy of This Notice
You have the right to receive a paper copy of this Notice at any time, even if you agreed to receive it electronically.

If you believe your privacy rights have been violated, you may file a complaint with us or with the U.S. Department of Health and Human Services, Office for Civil Rights. We will not retaliate against you for filing a complaint.
